How can I prescribe medications through MediVera Compounding Pharmacy?
You can easily prescribe via fax, phone, our Provider Portal, or your EMR.
Providers can prescribe medications to MediVera using any of the following methods:

Fax: Complete a fax form request to receive a fax form with the medications of your choosing. Then print and send the fax form to 866-531-1826.

EMR: Submit prescriptions directly through your EMR system. MediVera is available in most major EMRs — search for "MediVera Compounding Pharmacy" or contact us if you need assistance.

Phone: Call us at 877-531-1147 to speak with our team and place a prescription by phone.
